2020–2022年福州市8~10岁儿童碘营养状况及甲状腺容积影响因素分析

摘要

目的 了解福州市8~10岁儿童碘营养状况,并探讨儿童甲状腺容积与体格发育指标的关系。方法 采用多阶段分层抽样,2020年4月–2022年4月每年在福州市所辖12个县(市)区按东、西、南、北、中5个方位各随机抽取1个乡镇/街道的1所小学,每所小学抽取8~10岁非寄宿学生40人,采集儿童即时尿样进行尿碘检测,使用B超法测量甲状腺容积,测量儿童身高、体重,并对儿童体格发育指数与甲状腺容积的关系进行分析。结果 2020–2022年累计调查8~10岁儿童7390人。尿碘中位数为174.30 µg/L。共检查7390名儿童甲状腺,甲状腺肿大79例,甲肿率1.07%。儿童甲状腺容积范围为0.6~16.5 ml,中位数2.51 ml。8~10岁儿童甲状腺容积与体表面积、身高、体重、年龄及身体质量指数均成正相关。多重回归分析结果显示体表面积对甲状腺容积影响大于年龄。结论 福州市学龄儿童碘营养状况适宜,甲状腺容积随年龄、体格发育的增大而增大。

Abstract

Objective To investigate the iodine nutrition status of children aged 8-10 years in Fuzhou city, and to explore the relationship between thyroid volume and physical development indexes in children. Methods Multi-stage stratified sampling was adopted. From April 2020 to April 2022, a primary school in a township/street in 12 counties (cities) of Fuzhou was randomly selected from the east, west, south, north, and middle directions each year, and 40 non-boarding students aged 8-10 years were selected from each primary school. Instant urine samples were collected for urine iodine detection, and thyroid volume was measured by B-ultrasound. The height and weight of children were measured, and the relationship between children's physical development indexes and thyroid volume was analyzed. Results A total of 7390 children aged 8-10 years were investigated from 2020 to 2022. The median urinary iodine was 174.30 µg/L. A total of 7390 children were examined for thyroid gland. About 79 goiter cases were found, and the rate of goiter was 1.07%. The thyroid volume of children ranged from 0.6 to 16.5ml, with a median of 2.51ml. Thyroid volume was positively correlated with body surface area, height, weight, age, and body mass index in children aged 8-10 years. The multiple regression analysis showed that the influence of body surface area on thyroid volume was higher than that of age. Conclusion The iodine nutritional status of school-aged children in Fuzhou is suitable, and the thyroid volume increases with the increase of age and physical development.
