乳母膳食多样性与婴儿皮疹发生风险的关联

摘要

目的 探讨哺乳期妇女膳食多样性与婴儿1月龄皮疹发生风险的关联。方法 研究对象来自“关于母乳成分、喂养状况和婴儿健康的中国母婴队列研究 (Phoenix研究小组) ”。通过问卷调查获取乳母的社会人口学特征等信息;采用食物频率问卷(FFQ)和24h饮食日记调查膳食摄入情况;根据联合国粮农组织提出的育龄妇女最低膳食多样性(Minimum Dietary Diversity for Women,MDD-W)指南将所有食物条目分为10个食物组,各食物组得分数相加得到MDD-W评分;询问母乳喂养情况和1月龄的婴儿皮疹患病情况;采用Logistic回归模型分析乳母MDD-W评分与婴儿皮疹发生风险的关联,并通过分层分析探究全母乳喂养的潜在影响。结果 本研究最终纳入694对母婴,其中221(31.8%)名婴儿患有皮疹,平均MDD-W评分为5.9±1.5分,≥5分者占82.9%。调整混杂因素后,以母亲MDD-W评分≥5分组为参照,<5分组的婴儿患皮疹的风险显著增加(OR=1.96, 95% CI:1.27, 3.02),且在非全母乳喂养组中关联更加显著(OR=3.01, 95% CI:1.58, 5.72)。结论 我国哺乳期妇女膳食多样性水平较高,且乳母膳食多样性增加会降低婴儿患皮疹的风险。

Abstract

Objective To investigate the association between dietary diversity during lactation and the risk of infant rash at 1 month of age. Methods Participants in this study were from the Chinese mother-infant cohort study on breast milk composition, feeding status, and infant health (Phoenix Study). Basic characteristics of mothers were collected using the questionnaire, and dietary intake was assessed using validated food frequency questionnaires and 24-hour diet diaries. All food items were categorized into 10 groups based on guidance from the Food and Agriculture Organization of the United Nations to construct the Minimum Dietary Diversity for Women (MDD-W) score. The scores of all food groups were summed as the MDD-W score. Infant rash and breastfeeding status were recorded at 1-month postpartum visit. Logistic regression model was used to assess the association between maternal MDD-W score and the risk of infant rash. The potential impact of full breastfeeding was explored through stratified analysis. Results A total of 694 mother-infant pairs were included in this study, of which 221 (31.8%) with rash during the first month of postpartum. The mean MDD-W score of mothers was 5.9±1.5, and 82.9% mothers scored ≥5. After adjusting for confounding factors, maternal MDD-W score <5 was associated with the higher risk of infant rash (OR=1.96, 95% CI: 1.27, 3.02) compared MDD-W score≥5. Moreover, the association was more significant in the non-full breastfeeding group (OR=3.01, 95% CI: 1.58, 5.72). Conclusion The dietary diversity of lactating women in China is relatively high, and increased maternal dietary diversity during lactation will reduce the risk of rash in infants.
